This is a genuine Milwaukee replacement part, it is sold individually. The Milwaukee Belt Hook is attached to the bottom of the handle assembly on the tool model numbers shown below. Its purpose is to hook the tool to your belt when not using the tool. The Belt Kit includes the screw to attach it to the housing of the tool.
I received a Milwaukee drill driver and impact driver as a gift. The drill driver did not come with a belt clip to attach to your belt nor a bit holder. I attached the belt clip and bit holder on to the drill.
Remove the back cover
Remove the screws from the housings and carefully separate
Take a picture of where the internal part are placed
Start placing the parts inside of the new housing half
Check to make sure you moved all of the parts and they are in the correct place

Replaced speed selector button/lens on a Milwaukee 2653-20
