Hello Donnie, thank you for your question. The model number is on the charger is usually located on the side of the charging station, printed on a label attached to the plastic. The date code can be found at the bottom, and it will be easy to identify since it is engraved on the housing and has a date format. You only need to provide the first six digits. We hope this helps.
