I removed the blade bolt, lock washer and the damaged hardened washer and the blade. I then remove the damaged blade adapter. And installed the new one making sure the key in the adapter lined up with the keyway in the shaft. Height adjustment bracket was loose and was going to fall off the mower eventually.

I took the height adjustment bracket off the mower and replaced 3 parts. I was not sure which part was defective so I ordered 5 parts and 3 were used to replace the old parts and now it is tight and does not wobble anymore.
