The insert goes next to the retaining ring. This part is available as an individual replacement part. This is an original Porter Cable and Black & Decker part for use in routers. A set of pliers might be needed during the installation process. This part is made out of metal and will need to be replaced if it breaks.
No instructions other than the parts list and parts diagram provided on eRlacementparts web site. Thanks for a quick solution and a user friendly site for guys like me.

The aluminum clamp is too soft should someone try to unlock and already unlocked clamp. The same is true for locking an already locked clamp. It should be made of steel
1. Using star screwdriver removed large machine screw via hole in open clamp.
2. Removed insert and clamp fell off axle. Examined both pieces for wear that would explain the problem. Both looked pretty good.
3. Put axle in place. Slipped clamp over it and inserted machine screw part way.

5. Closed and opened clamp several times and adjusted tightness of machine screw. Can only access screw with clamp open so some trial and error.
6. Tested above table tool - didn\'t work. Realized problem was wear of hex shaped hole in clamp.
7. Ordered new clamp from eReplacementparts
8. Installed new clamp as above. Works perfectly.
