Parts Used:
90578994-26, 5140034-37, 5140034-36, 5140034-34
I installed the missing parts in order
1) screw on inner flange
2) put blade on
3) slide on the clamp
4) screw in the bolt hand tight
5) attach vise-grips so blade does not move, tighten bolt using adjustable wrench

For cord replacement
1) take off plastic covers using Phillips screwdriv er
2) take of old cord and attach new one to appropriate positive and negative sides
3) put plastic covers back on

my saw looked and worked like brand new
