Pedal Arm Sleeve was coming apart. I replaced both as well as the Large Axle Cover.
1. Removed both axle covers
2. Unhooked the arms from the pedal arm sleeves
3. Removed the sleeves
4. Pushed out the bearings on each side and placed them in the new sleeves
5. Used Teflon lube on both sides
6. Slid sleeves back on
7. Replaced axle covers on both sides
I only used a screwdriver, a rag, and some lubricant to do the job. Had a workout on the repaired machine and it worked great!

Pedal are sleeve and axle cover broke.
Remove pedal arm from arm sleeve. Used Allen head wrench to remove the bolt holding the axle cover and pedal arm sleeve on the axle arm. Slide pedal arm sleeve from axle arm, careful not to damage bearings on the axle sleeve. Use a screw driver to Carefully remove bearings from axle sleeve and inspect them to make sure they are in good working order. If the bearings are not in good condition, order new replacements before completing the repairs. Insert bearings in the new are sleeve the same way they were removed. Slide pedal arm sleeve onto axle arm, place axle cover onto the end of the axle sleeve and secure in place with bolt and washer. Clean any debris from the underside of the pedal arm then Re align pedal arm and secure in place.
