How do I tighten the blade? With the nut tight, the blade still spins on the shaft?

On my Skil Circular saw, Model 5350, my saw blade and the shaft moves in and out slightly by hand. The blade wanders too much when cutting. What can I do to remedy this? New Bearings? Shims?
